Transaction Procedures & Requirements

STEPS TO FOLLOW IN SECURING BIRTH CERTIFICATE
(HAKBANG SA PAGKUHA NG KATIBAYAN NG KAPANGANAKAN)

STEP 1: APPLICATION FORM/VERIFICATION AND INTERVIEW:
1.1 Secure an application form. Fill-up all the necessary information. Return the accomplished form to the assigned personnel on Step 1 for Verification.
(Kumuha ng application form at isulat ang mga kinakailangang impormasyon.
Ibalik ang sinagutang application form sa nakatalagang kawani sa Step 1 para sa kaukulang beripikasyon ng dokumento).
1.2 Proceed to the designated waiting area for verification and wait for your name/name of person in the application form to be called for the result.
(Magtungo sa nakalaang lugar para sa beripikasyon at hintaying tawagin ang inyong pangalan/pangalan ng dokumentong kinukuha)
1.3 Prepare the required Authorization Letter and Valid IDs if needed. Be ready for interview and your queries regarding your document to facilitate its process. Wait for the Order of Payment.
(Ihanda ang mga Valid IDs at Awtorisasyon kung kinakailangan. Maghanda para sa panayam at ilaan ang mga katanungan ukol sa inyong dokumento. Maghintay para sa kaukulang bayarin).

STEP 2: PAYMENT
2.1 Proceed to the Taxpayer’s Lounge – City Treasurer’s Office(Adjacent Bldg.) for payment.
(Magtungo sa Taxpayer’s Lounge – City Treasurer’s Office para sa pagbabayad)

STEP 3: SCHEDULING
3.1 After payment proceed to Scheduling Area of |City Civil Registry Office and submit the Application Form together with the Official Receipt.
(Pagkabayad, magtungo sa Scheduling Area ng City Civil Registry Office at ibigay ang Application Form kalakip ang Resibong binayaran).
3.2 Wait for the Claim Stub indicating the scheduled Time and Date of Release of the document
(Hintaying makuha ang Claim Stub na nakasaad ang araw at oras kung kailan makukuha ang dokumento).

P A A L A L A
Para sa mga kukuha ng dokumentong hindi sa kanila ay kinakailangang magsumite ng AUTHORIZATION LETTER kalakip ang VALID IDs ng may-ari ng dokumento at ng kumukuha, MALIBAN na lang kung ang kumukuha ay malapit na kaanak gaya ng magulang, kapatid, lolo at lola.

STEPS TO FOLLOW FOR LATE REGISTRATION OF BIRTH CERTIFICATE OF CHILDREN ATTENDED BY MIDWIFE / HILOT AND BORN IN HOSPITAL (MINOR, SINGLE, NOW MARRIED)

1. Latest Certificate of No Record from NSO and LCR of Manila
2. Endorsement from the Hospital and Affidavit stating reason of delay, if born in Hospital.
3. Affidavit from the Midwife and Hilot stating reason of delay and Midwife License, if attended by Midwife and Hilot
Item no.2- not required if the hospital is closed and the Midwife and Hilot is deceased and unknown
NOTE: Submit Pre-Natal Record of the mother if 40 y/o and above at the time
of the conception of the child regardless of birth order.
4. Certified copy of Marriage Contract of Registrant, if married
5. Certified copy of Marriage Contract of Parents
If no record was issued, submit Birth Certificate of brothers and sisters with date and place
of marriage of parents or Birth Certificate of older brothers and sisters.
If not married, submit an Affidavit to Use the Father Surname (AUSF)
Pursuant to RA 9255 and also to accomplish the Affidavit of Acknowledgement/Paternity
at the back of COLB (For Children born after August 1988)
Father to sign at the back portion of the Certificate of Live Birth for Admission of Paternity and Acknowledgement (For Children born before August 1988)
If Father is deceased, submit documents such as Insurance, ITR (Income Tax Return) and other records that will prove the filiation of the child or documents showing that the father has acknowledged the child.
Submit original copy of any two (2) of the following documents with the date and place of birth (For Registrant)
*Baptismal Certificate
*School Records showing full name as registered in the school, date and
place of birth, name of parents (Either Elementary or High School)
*Transcript of Record with date and place of birth
*Voter’s Affidavit
*Employment Records (SSS/GSIS), if employed
*ITR (Income Tax Return), if employed
*Pre-Natal Record (FOR MINOR CHILDREN)
*Other documents the office may consider relevant and necessary for the
approval of the application (Philhealth, Service Record, Personal Data Sheet, Medical Record, Certification from OSCA.etc.)

REMINDERS:
1. Don’t submit “FAKE DOCUMENTS” to avoid penalty. All supporting documents are subject for Verification.
2. All Birth Certificate and Marriage Contract for submission must be certified photocopy.
3. If Birth Certificate is prepared by Midwife, Hilot and Hospital, please avoid erasures. Birth Certificate with erasures shall be returned for retype.
4. Please complete all the needed requirements before submission.
5. Ten (10) days posting period. Will commence on the day when the requirements was submitted.
FEES:
0-6 months (115.00)/6 months – less than 1 year (140.00) / more than 1 year (190.00)- For Children born in the hospital
For Children attended by Midwife/Hilot and born in Hospital but already closed (290.00) with AUSF additional payment of Php 30.00.

NOTE: NO RECEIVING OF APPLICATION AND NO FOLLOW-UP EVERY FRIDAY

STEPS TO FOLLOW AND REQUIREMENTS FOR CORRECTION OF CLERICAL ERROR IN THE FIRST NAME OF DOCUMENT OWNER (MARRIED) UNDER RA 9048

1. 2 latest certified/local copies and 2 latest NSO copies of birth certificate to be corrected.
2. 2 latest certified copy of marriage contract of the document owner.
3. 2 latest certified copies of birth certificate of at least 2 children of the document owner.
4. 2 copies of baptismal certificate.
5. 2 copies of school records, (Elementary and High School either F-137/138 or Certification) or College (TOR)
6. 2 certified copies of voter’s registration record/voters affidavit. (COMELEC)
7. 2 latest original NBI Clearance. (PURPOSE: FOR CORRECTION OF FIRST NAME)
8. 2 latest original PNP Clearance. (PURPOSE: FOR CORRECTION OF FIRST NAME)
9. 2 copies of valid ID of the petitioner and the document owner and 1 copy of latest community tax certificate from the place of work or residence.
10. Other documents which the Office may consider relevant and necessary for the approval of the Petition.
(GSIS/SSS Record, Diploma (elem., HS or College/Vocational), Medical Record, Business Record, Service Record, Insurance, Certificate of Land Title, Passbook etc.
11. SPA (Special Power of Attorney), If the petitioner is abroad, or sick, he/she can be represented by lawyer or his/her nearest relative (up to third degree of consanguinity).

REMINDER:
1. All civil documents (Birth, Marriage and Death) to be submitted should be the latest certified local copy or Security paper from NSO
2. After the compliance of the requirements, please proceed to the information counter and get a number for the Pre-interview and bring the original copies of the supporting documents (Personal Records)
(Only with Complete requirements are allowed for pre-interview)
3. Steps to follow will be provided after the Final Interview
4. Processing of the petition is four (4) months. Will commence on the date the petition will be receive by this Office
5. Payments are as follows: Registration Fee (P 1,000.00)/Certified Xerox Copy (P230.00) and Transmittal Fee (P 210.00) and Additional Payment of 30.00 (Document with supplemental report.

1. 2 latest certified/local copies and 2 latest NSO copies of birth certificate to be corrected.
2. 2 copies of baptismal certificate.
3. 2 copies of school records (Elementary and High School either f-137/138 or Certification) or College (Transcript of Records).
4. 2 certified copies of voter’s registration record/voters affidavit. (COMELEC)
5. 2 latest original NBI Clearance. (purpose: FOR CHANGE OF NAME)
6. 2 latest original PNP Clearance. (purpose: FOR CHANGE OF NAME)
7. 2 copies of valid ID of the petitioner and the document owner and 1 copy of latest community tax certificate from the place of work or residence.
8. Other documents which the Office may consider relevant and necessary for the approval of the Petition
(GSIS/SSS Record, Diploma (Elem., HS or College/Vocational), Medical Record, Business Record, School Record, Service Record, Insurance, Certificate of Land Title, Passbook, etc.
9. SPA (Special Power of Attorney), if the petitioner is not the document owner like auntie, uncle, godparents, client, friend, officemate, etc.

REMINDERS:
1. All civil documents (Birth, Marriage, and Death) to be submitted should be the latest certified local copy when issued in Manila, if issued outside Manila, Security Paper from NSO.
2. Submit all requirements to R.A. 9048 receiving table for assessment and initial interview.Please bring all original documents and ID. (Table 1)

STEPS TO FOLLOW AND REQUIREMENTS FOR CORRECTION OF DATE AND PLACE OF MARRIAGE IN THE BIRTH CERTIFICATE OF THE DOCUMENT OWNER UNDER RA 9048

1. 2 latest certified local copies and 2 latest NSO copies of birth certificate to be corrected.
2. 2 latest certified copies of marriage contract of parents.
3. 2 latest certified copies of birth certificate of brother and sister of the document owner at least two (2).
4. 2 latest copies of CENOMAR of both parents from NSO
5. 2 latest certified copies of parent's marriage contract from the church stating the date of the civil marriage and /or certificate of no record of parent’s marriage contract in reference to the erroneous date and place of marriage indicated in the birth certificate to be corrected
6. 2 copies of baptismal certificate.
7. 2 copies of school record, Elementary and High School either F-137/138 or Certification or College (TOR)
8. 2 copies of valid ID of the petitioner and the document owner and 1 copy of latest community tax certificate from the place of work or residence.
9. SPA (Special Power of Attorney), If the petitioner is abroad, or sick, he/she can be represented by lawyer or his/her nearest relative (up to third degree of consanguinity).

REMINDERS:

  1. 2 latest certified LCR copies and 2 latest NSO copies of birth certificate to be corrected.
  2. 2 latest certified copies of marriage contract of document owner, if married
  3. 2 latest certified copies of certification from the hospital indicating and the exact address of the hospital/If the hospital or Lying Inn Clinic was already closed; please submit certification from the Barangay stating that the said hospital or Lying Inn Clinic was formerly established in their place and was already closed.
  4. 2 copies of baptismal certificate.
  5. 2 copies of school record, Elementary and  High School either form 137/138  or Certification or College (TOR)
  6. 2 Certified copies of voter’s registration record/voters affidavit (COMELEC)
  7. 2 copies of valid ID of the petitioner and the document owner and 1 copy of   latest  community tax certificate from the place of work or residence.
  8. SPA (Special Power of Attorney). If the petitioner is abroad, or sick, he/she can be represented by lawyer or his/her nearest relative (up to third degree of consanguinity).

  1. 2 latest certified LCR copies and 2 latest   NSO copies of birth certificate to be corrected.
  2. 2 latest certified copies of birth certificate of all   brothers and sisters of the document owner.
  3. 2 latest original or certified copies of Obstetrical record, Medical record and Pre-natal record from the hospital and/or OB GYNE
  4. 2 Xerox copies of any document/s (SSS, GSIS, BIR, PHILHEALTH, PRIVATE INSURANCE, etc.)of the parents where all their children are indicated as their beneficiary and arrange accordingly    
  5. 2 copies of valid ID of the petitioner and the document owner and 1 copy of   latest   community tax certificate from the place of work or residence.
  6. SPA  (Special Power of Attorney), If the petitioner is abroad, or sick, he/she can be    represented by lawyer  or his/her nearest relative (up to third degree of consanguinity).

STEPS FOR AUSF PERMISSION TO USE THE SURNAME OF THE FATHER and Requirements for RA 9255

  1. Latest Certified Copy of Birth Certificate of the Child. Include dorsal page if signed by father.
  2. Valid ID of Father and Mother (Original and Xerox Copy). CEDULA of Father.
  3. Original and Photocopy of Baptismal Certificate.
  4. Original School Records showing the names of the parents (Must be Certified True Copy).
  5.  Both Parents must come for interview.
  6. If mother is deceased, submit Certified Copy of Death Certificate (Latest Copy).
  7. If child is of legal age (18 yrs. Old and above)
  • Submit Valid ID
  • For interview together with both parents.
  • Submit all the above requirements (nos. 1-6)
  1. If Filiations has not been expressly recognized by the father, submit any two (2) of the ff. documents showing clearly the paternity between the father and the child:
  • Employment Records
  • SSS/GSIS Records
  • Insurance
  • Certification of Membership in any organization
  • Statement of Assets and Liabilities.
  • Income Tax Return (ITR)

I.   SUBMIT THE FOLLOWING DOCUMENTS (ALL ORIGINAL COPY) AT RM. 117-A.

1.   Latest Local Certified True Copy of birth Certificate of the child.

5.   If the name of the father does not appear at the birth certificate of  the child Submit at  least 2 supporting documents from the following:

5.2   GSIS of the father where the child is declared as beneficiary.

5.3   Income Tax Return of the father where the child is declared as beneficiary

5.6   Original Copy of Report card  (F-138) & Certified True Copy of School Record  (F-137)

        of the child wherein the name of the father is declared and the signature of the father

5.7   Statement of assets and liabilities

6.   If mother is deceased, submit the Latest Certified True Copy of Death Certificate.

7.   If father is deceased, AUTHENTIC WRITING WILL BE NEEDED. SUBMIT THE FOLLOWING        DOCUMENTS LISTED AT NUMBER 5 or any other records that will prove the filiation of the   child or documents showing that the father has acknowledged the child.

REQUIREMENTS FOR THE CORRECTION OF SEX OR GENDER UNDER RA 10172

  1. Document owner shall file the petition themselves except in cases when the document owner is a minor or physically or mentally  incapacitated, the petition may be filed on his/her behalf by his/her spouse, or any of his children, parents, brothers, sisters, grandparents, guardians, or persons authorized by law. However, APPEARANCE OF THE DOCUMENT OWNER SHALL BE REQUIRED.
  2. One (1) latest certified machine copy and one (1) photo copy of the birth certificate to be corrected issued by the Manila  Civil   Registrar.
  3. One (1) latest NSO Copy and one (1) photo copy of the birth certificate to be corrected.
  4. Medical Certification issued by the Manila City Health Officer as to the true gender of the document owner and same has not undergone sex change or sex transplant. Please get your referral slip at the Administrative Division and look for Ms. DEL NAVATA and proceed to the Office of the Manila City Health Officer, Manila Health Department   located at Room 132, Ground Floor, City Hall Manila. Payment for the medical is Php 100.00 only.
  5. Ultra Sound and one (1) colored whole body picture.
  6. One (1) original and one (1) photo copy  of  the  following MANDATORY REQUIREMENTS

 

-          Elementary school record (Form 137/school certification).  In case the petitioner/document owner never entered school, an Affidavit attesting to the facts shall be submitted
